Applications are taken every Wednesday between 9 a.m. and 11:30 a.m. and 1 p.m. and 3 p.m. at 320 E. West Mine Street, Hazleton, PA 18201.

Pre-Application Process

You must bring all the following documents for every member of the household to complete the application.

  1. Current proof of residence (Head of Household).
  2. Original Birth Certificate.
  3. Passport.
  4. Drivers License or State ID.
  5. Permanent Resident Card (if not a U.S. citizen).
  6. Social Security card.
  7. Proof of income.
